Insurance

 

We will submit claims to your insurance as a courtesy to you. Please be aware that insurance does not cover all procedures 100%. It is your responsibility to know your insurance coverage and policy limitations. You are ultimately responsible for your bill if your insurance does not pay in a timely manner. Any insurance plans that pay patient directly will be treated as private pay. Please let us know if you require a printout to submit to your insurances.

 

Preauthorization's

Preauthorization's are done for most major procedures if time allows. This is a estimate only and is not a guarantee of benefits.

 

We must have current and correct information to process claims. Please report any changes of insurance, address or phone number to ensure your insurance will pay.

 

Payment Options

We accept cash, checks, Visa, Mastercard, American Express and Discover. We also participate in the CareCredit program which you may apply for online or in our office.

 

We offer a discount for patients with no insurance. Services paid in full on the day services are rendered, are eligible for a 10% discount if paid by cash or check.

 

Financial arrangements may be available for some services. Please contact our Accounts Receivable Department prior to beginning any work to discuss your options. Major work such as crowns, bridges, dentures, partials or implants requires full payment of your anticipated copay when work is started.

 

Missed Appointments

Our office understands that from time to time patients may miss their appointments. We reserve the right to charge you a nominal office visit charge for any appointments not kept or canceled with less that 24 hours notice.
